• Eligibility

I confirm my child spend time on a neonatal unit, special care baby unit and/or transitional care in the North East.

• Attendance and Childhood Illnesses

We kindly ask that you are sensitive to the needs of all families attending and do not attend if you or your children are showing any signs of illness.

Upon signing up for our sessions, you acknowledge the risk of contracting common viruses and childhood illnesses that can be passed easily through children.

If your child becomes unwell after attending a session, we kindly recommend seeking advice from your GP. Please wait until your child is feeling better before attending another session.

• Attendance Policy

I will do my best to let Tiny Lives know as soon as possible if I’m unable to attend a session.

To help ensure places are available for everyone, I understand that if three sessions are missed in a term without notice, my booking may need to be offered to another family.

• Data Sharing

I understand my details will be shared with the facilitator for their records
